EFFECTIVE IMMEDIATELY:

ALL IV HYDRATIONS TREATMENTS REQUIRE BASELINE LABS PRIOR TO INFUSION FOR PAST AND NEW CLIENTS FOR AN ADDITIONAL COST OF $35. THIS IS A ONE TIME LAB FEE UPFRONT FOR THE INITIAL TREATMENT. ADDITIONAL LABS WILL BE COLLECTED EVERY 90 DAYS (3 MONTHS).

BASELINE LABS AND EKG WILL BE REQUIRE PRIOR TO PHENTERMINE PRESCRIPTION FOR ADDITIONAL COST.

FORMS OF PAYMENT: CASH, CREDIT CARDS, AND FSA/HSA. IT IS THE RESPONSIBILITY OF THE CLIENT TO KNOW WHICH SERVICES OR PRODUCTS ARE COVERED UNDER FSA/HSA.

APPOINTMENTS CANCELLED LESS THAN 24 HOURS NOTICE AND ALL NO SHOWS, WILL BE CHARGED A CANCELLATION FEE OF FULL TOTAL AMOUNT OF VISIT.

UNDER NO CIRCUMSTANCES WILL A REFUND BE ALLOWED FOR ANY SERVICES RENDERED OR PURCHASED AT MY HEALTH REASSURANCE INCLUDING BUT NOT LIMITED USING FSA OR HSA AS FORMS OF PAYMENT FOR SERVICES.

RESERVATIONS

My Health Reassurance IV Hydration and Wellness and MHR Luxury Essentials are available by appointment on a daily basis. We recommend calling ahead to ensure availability. A credit card or gift certificate is required to reserve all health and beauty services. For parties of 5 or more a managing and gratuity fee will be added to regular service price.

GIFT CARDS, E-GIFT CARDS

Gift Cards may be redeemed for services or retail products and must be presented at the time of the appointment or retail purchase. Gift Cards will not be replaced if lost or stolen and will not be refunded. Gift Cards do not expire.

HEALTH AND BEAUTY INFORMATION

We suggest that you arrive 10 minutes before your appointment; if you are a first-time client, a profile form will be completed prior to service.

LATE ARRIVALS

We regret that late arrivals will not receive an extension of scheduled service times and will be responsible for full service fees.

JEWELRY

We suggest you arrange for your valuables prior to your visit with us.

CELL PHONES

Out of consideration for all guests, please mute cell phones. Cell phone use is not permitted during a treatment or service. Our front desk staff will assist you if needed.

CHILDREN & TEENS

The minimum age requirement for services is defined by local, city and state ordinances. Children under 12 are not permitted in service area or treatment rooms. Please arrange for childcare prior to visit.
